AV5Why™ is not another version of traditional Five Why. It is a structured causal reasoning system designed to make investigative reasoning explicit, reviewable, challengeable, and progressively validated.
What makes it different
Rather than allowing an answer to automatically become the next question, AV5Why requires each causal claim to earn that right. Evidence, mechanism, assumptions, competing explanations, challenge, and validation are made visible as the investigation progresses.
Cause Verification Records
AV5Why uses Cause Verification Records (CVRs™) to capture and evaluate causal reasoning as a controlled investigative artifact. This creates transparency and allows teams, reviewers, leaders, customers, and eventually AI systems to understand why a causal conclusion was accepted.
Designed to complement existing systems
AV5Why can strengthen investigations conducted within broader problem-solving and corrective-action frameworks. Its role is to improve causal reasoning, not to replace the surrounding management system.
